Dependent Documentation

If you add a dependent to your Medical, Dental, Vision, Accidental Injury, Critical Illness, Hospital Care, Spouse/Partner Life or Child Life coverage(s), you must document the dependent's relationship to you and the dependent's birth date by providing dependent documentation as noted below. You will also be required to provide your dependent's social security number or tax identification number within 90 days of the qualifying event date.

Enrolling A Spouse

To enroll your spouse, you must provide:

  • A copy of your marriage certificate* and
  • A copy of your spouse's birth certificate, passport, driver's license, or some other government-issued document that shows his or her date of birth

*Your required dependent documentation must be provided within 60 days of your marriage date.

Enrolling A Domestic Partner

To enroll your domestic partner, you must provide:

  • Domestic Partnership Agreement or Civil Union Partnership Agreement filed with the appropriate authority in your state of residence, or
  • GEICO's Affidavit for Domestic Partner or Civil Union Partnership notarized AND Domestic Partnership Agreement or Civil Union Partnership Agreement not on file with any state authority, or
  • GEICO's Affidavit for Domestic Partner or Civil Union Partnership notarized PLUS two forms of dependent documentation from GEICO affidavit list (A-N) located here.
  • (if the associate does not have a Domestic Partnership Agreement or Civil Union Partnership Agreement filed with the appropriate authority in your state of residence).

*Additionally, a copy of your partner's birth certificate, passport, driver's license or some other government-issued document that shows their date of birth is required, if not listed on your certificate.

Enrolling A Child

To enroll your child, you must provide:

  • A copy of the child's birth certificate** (it must contain the associate's name or the associate's spouse's name), or
  • A court order awarding custody or guardianship, if applicable, or
  • Evidence of adoption, if applicable

**If the child is newly born or recently adopted and you do not have a birth certificate, you may provide temporary documentation, such as a copy of the souvenir birth certificate from the hospital or a copy of the mother's hospital discharge paperwork, within 60 days of the child's birth/adoption. You must provide the official birth certificate within 60 days of the child's birth/adoption.
