Supporting Documentation
If you change your benefit elections due to a qualifying event, you must provide supporting documentation as shown in the chart below. Except as noted, all documentation must be submitted within 60 days (30 for New Hire Event) of the qualifying event date.

Birth/Adoption | You must submit a copy of one of the following documents within 60 days of your child's birth/adoption: child's birth certificate*; a court order awarding custody or guardianship, if applicable; or evidence of adoption, if applicable. You must also provide your child's social security number or tax identification number within 90 days of the child's birth/adoption date. Your name or your spouse/partner's name must be listed on the child's birth certificate. *If the child is newly born or adopted and you do not have an official birth certificate, you may provide temporary documentation, such as a copy of the souvenir birth certificate from the hospital or a copy of the mother's hospital discharge paperwork, within 60 days of the child's birth/adoption. You must provide the official birth certificate within 60 days of the child's birth/adoption. |
Change Voluntary Insurances | None required. (To enroll in Spouse or Child Life coverage, dependent documentation is required) |
Death of Dependent | Copy of dependent's death certificate, obituary or police accident report. |
Divorce/Dissolution of Partnership |
Spouse: Copy of divorce decree signed by a judge, including pages with the names of the associate and spouse, divorce date and judge's signature. Pages related to the division of marital assets and custody arrangements are not required. Domestic Partner: Copy of Domestic Partner Termination Affidavit signed and notarized. |
Gains Eligibility | Copy of documentation of associate's or dependent's enrollment in another employer's benefit plan (medical, dental, vision or flexible spending account), Medicare or Medicaid plan. Examples include: annual enrollment paperwork from another employer that shows enrollment period start and end dates; written confirmation of enrollment in another employer's benefit plan (medical, dental, vision or flexible spending account) with associate's or dependent's name and effective date; or letter from Medicare or Medicaid that shows the name of the associate or dependent and the enrollment effective date. |
Loses Eligibility | Copy of documentation of associate's or dependent's loss of coverage in another employer's benefit plan (medical, dental, vision or flexible spending account), including termination of employment, end of COBRA coverage, or loss of coverage in a Medicare or Medicaid plan. Examples include: annual enrollment paperwork from another employer that shows enrollment period start and end dates; written confirmation of loss of coverage in another employer's benefit plan (medical, dental, vision or flexible spending account) with associate's or dependent's name and effective date; or letter from Medicare or Medicaid that shows the name of the associate or dependent and the coverage loss effective date. |
Marriage/Enrolling a Domestic Partner |
Spouse: Copy of an offical marriage certificate provided within 60 days of your marriage date and a copy of your spouse’s birth certificate, passport, driver’s license, or some other government-issued document that shows his or her date of birth. You must also provide your spouse's social security number or tax identify number within 60 days of your marriage date.
